Simplified Practice | User Data Deletion

1. Submit a deletion request

Email info@simplifiedpractice.com with the subject line Meta Data Deletion Request.

Include only the information we need to find and verify your account:

Your full name and the email address associated with your account.
Your clinic or business name, if applicable.
Your Facebook or Meta user ID, if you know it.
A brief statement asking us to delete data received through the Meta integration.

Please do not send patient records or health information by email. We will contact you if we need additional information to verify the request.

2. We verify the request

To protect your information, we may ask you to confirm control of the email address or account connected to the integration. We will use any verification information only to authenticate and process the request.

3. We disconnect and delete the data

After verification, we will disconnect the applicable Meta integration and delete or de-identify personal data we received from Meta that is associated with your account, such as a Meta user identifier, profile details, and integration metadata, where that data is stored by Simplified Practice.

We generally complete verified requests within 30 days. We may retain limited information when required for security, fraud prevention, legal compliance, or to document that we fulfilled the request. Data in protected backups may remain until those backups expire under our normal retention schedule.

4. Remove Simplified Practice from Facebook

You can also remove the connection from your Facebook account by opening Settings & privacy, selecting Settings, then Apps and websites, and removing Simplified Practice. Removing the app stops future access but may not delete data already received, so please submit the email request above if you also want that data deleted.

5. Data managed by a clinic

If a clinic or healthcare practice submitted your information to Simplified Practice, contact that clinic first. The clinic controls that information and may need to instruct us to delete it, subject to applicable record-retention requirements.
